Immigration Questionnaires

This page is for international students who have been admitted into a full-time program at Georgetown University.  If you have not yet been accepted to Georgetown University, please contact the appropriate admissions office. 

All admitted non-U.S. citizens (including U.S. Permanent Residents) must complete the Immigration Questionnaire (IQ).  Students applying for an F-1 or J-1 visa must also provide additional documentation in order to facilitate the issuance of an I-20 or DS-2019 Form for the visa interview.  Please click on the appropriate link below to download the questionnaire.
